We seek to deal carefully with the personal information we collect and to do so in a manner consistent with your reasonable expectations. The personal information in this form will be made available to Emmanuel Church Ministry and Admin staff and leaders involved in the running of the activities in which your child participates, and medical and emergency services if considered necessary.

Permissions

Permission for photos/video *

I recognise that registering my child for a program or event at Emmanuel Anglican Church implies consent for my child to be captured in photos and videos and understand that any such material will be used responsibly by the church, including for promotional purposes. We respect the right of parents/guardians to choose how their child’s images are used. Should you wish for photos/videos of your child not to be used, please let us know by emailing info@emmanuelchurch.org.au. If you ask for images of your child not to be used in this way, every reasonable effort will be made to avoid taking photos of your child, but we cannot guarantee that no photos will be taken. However, we will screen photos prior to use to ensure that your child is not included

Medical treatment *

Medical treatment: I recognise that registering my child for a program or event at Emmanuel Anglican Church implies that I give permission for medical aid to be sought for my child in the event of any accident or illness. I understand that every reasonable effort will be made to communicate with me before seeking medical attention for my child and I accept responsibility for any reasonable expenses incurred.

Supervision *

I understand that under the direction of the Senior Minister, Kids Ministry Co-ordinator and Assistant Minister, employees and volunteers of Emmanuel Anglican Church will provide supervision during programs ensuring all relevant Government Child Protection legislation requirements are followed, and that I can find more information Safe Ministry on the Emmanuel Church website. Supervision will only be provided during the specified session times of the program. Outside of these times I understand that it is my responsibility to supervise my child/ren.

Gecko Walking Bus

I give permission for my child to be collected by Gecko Kids Club leaders from Glenhaven Public School and join the walking bus to Emmanuel Anglican Church Glenhaven. Please note the walking bus departs with all present children at 3:05pm.
